Outcare is seeking two people to fill the role of Case Manager - Aboriginal Throughcare.
Successful candidates will be responsible for providing ongoing support and expertise for the planning and implementation of person-centered action plans that fulfill the reintegrating and re-connection of clients who have been or are at risk of being detained within the justice system.
A tertiary qualification in a relevant discipline (Social Work, Community Services, Criminology) or relevant experience working in similar roles is key for these postitions.
