Moorditj Koort Aboriginal Corporation (MKAC) is recruiting for a Care Coordinator (Aboriginal Health Practitioner).
The Care Coordinator (Aboriginal Health Practitioner) arranges the required services outlined in the client’s care plan, works collaboratively and connects clients to a range of health professionals, and maintains client records in the MKAC client data system.
It’s a full-time role based in Midland.
Applicants must have a Aboriginal Health Practitioner Cert IV or equivalent health qualification e.g., nursing, a valid WA Driver’s license, and AHPRA registration.
