Moorditj Koort Aboriginal Corporation is seeking a Care Coordinator for a full time position based in Kwinana.
Moorditj Koort Aboriginal Corporation is an Aboriginal community controlled, not-for-profit organisation supporting the health and wellness of Aboriginal and Torres Strait Islander peoples living in Western Australia.
The Care Coordinator works with clients to arrange required services outlined in the client’s care plan, connect them to health care professionals, identify gaps in specialist care, and arrange transport to and from appointments.
The annual salary is $65,000 – $70,000 per year, plus salary packaging.
