It’s a full-time role based in East Perth. The annual salary range is $124,843.68 to $140,625.20 plus superannuation and salary packaging.

Karla Kuliny is seeking three (3) Transition Coaches - Home Stretch Program.
This role provides one to one support, advice, and service coordination for young people transitioning from Out-of-Home-Care, and makes a difference in the lives of young people by offering an extension of support and resources to the age of 21 years.
Transition Coach’s will provide guidance and support to young people in the program, supporting them to strengthen and maintain their connections to culture, country, family, and community.
Develop young people's natural networks, ensuring they have a strong support system around them, and support young people by brokering access to essential financial resources and supports through the Home Stretch WA service, offering a reliable safety net to young people in the program. Provide young people with tailored supports utilising a culturally safe and strengths-based approach.
The role works with the Department of Communities’ Child Protection staff, foster families, carers, and the young person’s kin and community. Transition Coaches will have an approach that is trauma-informed and culturally secure.
There are three (3) full-time opportunities for an initial fixed-term contract until 16 September 2026, based in East Perth.
